District community nurse - #2154967

As a Band 5 District Community Nurse, you will be providing high-quality nursing care to patients within the community.

Key responsibilities will include:

• Completing patient assessments and developing individualised care plans
• Delivering and evaluating nursing care and treatment plans
• Administering medication
• Wound care and dressing management
• Continence care
• Supporting patients with long-term conditions
• Promoting independence and self-care
• Managing and prioritising a community caseload
• Working closely with GPs, therapists, social workers and other healthcare professionals
• Maintaining accurate clinical records
• Supporting safe and effective discharge and ongoing community care
• Working in line with clinical governance and evidence-based practice

What We’re Looking For We are looking for experienced Band 5 Community Nurses who are confident working independently within a community setting.
You should have:

• Active NMC registration
• Previous District or Community Nursing experience
• Strong assessment and care-planning skills
• Experience managing your own caseload
• Good communication and MDT working skills
• Ability to work autonomously in the community
