Job Overview

The formation of a stoma is a life-changing event. Therefore, expert specialist stoma care support is required not just in the acute post-operative period but in the long term to enable full rehabilitation and independence.

The Stoma Team cares for patients before and after emergency/elective colorectal and urology surgery. This is a diverse role involving a significant amount of clinical care, which is delivered within acute/community hospitals, nurse led clinics, patient’s homes and nursing/residential homes. Advice is also provided by telephone to patient, carers and GPs.

We have a commitment to working within the multidisciplinary setting with strong links to colorectal and uro-oncology teams and supporting the enhanced recovery protocol.

The successful applicant must show a significant interest in the speciality, and wound care, possess good communication and interpersonal skills and enjoy teaching, which is a major focus of this role. Previous experience in stoma care, surgery or gastro-enterology specialities is essential.

The role offers funded specialist training opportunities such as Masterclasses and practical study. Key board skills are essential.

Main duties of the job

The post holder will work as part of a team in providing specialist advice and nursing care for patients in the acute/community setting and nurse led clinics.

Provide support for patients undergoing stoma surgery for both benign and malignant disease and to assist in the adjustment to altered body image and function.

Promote patient independence in stoma care, optimising potential for recovery and rehabilitation to as near normal lifestyle as possible.

Acts as a resource for health care professionals, participating in teaching of patients and staff in clinical areas, aiming to improve and maintain quality standards of care for all stoma patients.

Detailed Job Description And Main Responsibilities

Clinical

Provides specialist clinical advice for patients, their families and health care professionals

  • Asseses, plans and implements specialist knowledge of relevant disease and treatment processes and stoma care products
  • Uses supporting skills to assist patients in adjusting to their disease, altered body image/function and potential sexual dysfunction
  • Promotes self-care by teaching patients wherever possible to manage their own stoma care
  • Manages a designated group of patients under the supervision of senior nurse and participates in nurse led clinics
  • Provides clinical advice on the use of stoma products for enterocutaneous fistulae
  • Refers to and liaises with other health care agencies, maintaining close links with Colorectal Uro-oncology and Palliative care teams
  • Provides prescribing information for General Practitioners and Community nurses
  • Maintains accurate patient records ensuring confidentiality is assured
  • Demonstrates evidence based practice
  • Demonstrates awareness of health and safety guidelines attending training sessions as appropriate e.g. Fire, Manual handling, Child protection
  • Ensures that Trust policies and procedures are followed

Education

Acts as a specialist resource for up to date information and advice for health care professionals in the acute Trust and Primary Care

  • Provides high quality health education/promotion for patients and families including diet, exercise, travel, return to work, hernia prevention and support
  • Provides up to date knowledge of current trends in surgical procedures and treatments, utilising research findings
  • Promotes quality standards of care and identifies educational requirements of staff caring for stoma patients
  • Promotes a positive learning environment within clinical settings
  • Acts as a role model within clinical practice
  • Participates in research & audit
  • Participates in the clinical governance agenda
  • Identifies own personal development/training requirements, including attending conferences/ seminars/ courses

Management

  • Contributes to service development and implement innovative ways of working
