Job Overview

NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly too.

The service uses a mixture of face-to-face visits, phone and video calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.

NHS@Home has six pathways, covering a wide range of conditions including frailty, respiratory, heart failure, and general surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration.

The service is run in partnership between North Bristol Trust, Sirona care & health, University Hospitals Bristol and Weston NHS Foundation Trust and BrisDoc.

Working in our team you will learn a very wide set of acute care skills in a community setting. You’ll also contribute to a multi-disciplinary approach, undertaking board rounds and working alongside consultants, advanced care practitioners, nurses and therapists to devise effective treatment plans that make a real difference to the lives of local people.

Main duties of the job

Working within the NHS@Home service as a band 6 Sister/Charge Nurse, your duties will include:

  • Coordinating the daily operations of the NHS@Home service from NBT base.
  • Having an oversight of all patients being supported under the service.
  • Supporting team members undertaking patient visits and make autonomous decisions regarding patient management plans.
  • Undertaking accurate patient assessments within in-patient and out-patient settings.
  • Supporting in-patient identification and manage referrals.
  • Oversee system-wide service capacity.
  • Assess patients and make comprehensive reports to the relevant MDTs.
  • Support Clinical Operations Lead and Senior Sister with the development, implementation and delivery of new clinical pathways and care initiatives to support service growth
  • Escalating clinical concerns appropriately.
  • Working autonomously making effective decisions.
  • Undertake community visits to deliver clinical care.

The post holder will work in collaboration with the NHS@Home team at Southmead hospital and BNSSG system partners at Sirona and UHBW. Hours of operation are 0700-21:30

It is essential you hold a valid full UK driving license and have access to an insured car with business insurance. Travel expenses will be paid.

Education/Training/Qualifications

Essential criteria

  • RGN with current UK recognised, live Registration
  • Full UK driving licence

Desirable criteria

  • Teaching and assessing qualification

Work Experience

Essential criteria

  • A current, acute and varied clinical background.
  • Understanding of the acute hospital processes relating to the in-patient pathway and discharge process.
  • Experience of working with multi-disciplinary teams.
  • Appropriate post registration experience.

Desirable criteria

  • Knowledgeable experience of audit.
  • Experience of service development.

Knowledge

Essential criteria

  • Is competent in a range of clinical skills
  • I.T skills to include Microsoft Word, Excel, Microsoft Outlook Email.

Personal Qualities

Essential criteria

  • Able to demonstrate initiative and work autonomously
  • Team leader, able to relate to all grades of staff.
  • Able to work and remain calm under pressure.
